Fresh urine doesn’t irritate your baby’s skin but if it sits for too long in his nappy, or on his skin, bacteria from his stools will break it down into ammonia. Ammonia is an irritant, it burns the skin creating the rash. Ammonia dermatitis usually appears before three months of age and is less common in breastfed babies.

It can appear where the nappy rubs against the skin, this is common in newborn babies. A friction rash can also occur where skin rubs together, naturally, this is more common in babies who are chubby.
